Pain, Discomfort, Hurt and Soreness?

I had a Twitter follower ask some questions about one of my Retweets concerning pain and rehab.
The basic gist of the tweet I retweeted was from @enahncedphysper and said “ your rehabilitation process should almost always be a pain dictated process” .
I 100% agree and my response was the title of one of my favorite articles, just think “ Does It Hurt”
The reply from @teir1athletics was “ isn't there going to be some discomfort though? I recall when I was working through my ROM on my knee ( post ACL) after surgery there was some discomfort? Should we educate the athlete on the difference between pain, discomfort, hurt and soreness?”
As I thought about the question I came to the conclusion that yes, we should educate our clients and athletes about the difference between pain, discomfort, hurt and soreness.
The reality is that there is a bit of subjectivity to the ideas of pain, discomfort, hurt and soreness. I'll give you what I think are some good working definitions as they apply to rehab and exercise.
Pain is pretty simple. Wikipedia defines pain as a distressing feeling. I feel like pain is easy to understand even if it can sometimes be hard to describe. Painful exercise is always wrong (unless we are talking about tendinopathy and, that's a different post).
(Also, please don't give me all the “pain science” BS. I realize pain is not simple but, that has nothing to do with painful exercise.)
Now, we get into semantics when we discuss discomfort. I do think that discomfort is different than pain but probably has a lot more latitude? What really constitutes discomfort? Discomfort is a lack of comfort? I think we want to generally avoid discomfort also. Discomfort is probably pains next door neighbor and discomfort can quickly become pain. This is the gray area. I know that squatting after knee surgery ( particularly before all the swelling is gone) can be a bit uncomfortable but is probably OK, as long as it doesn't progress to pain. I've often found that discomfort decreases with movement while pain increases?
Third, we get to the word hurt. When I think Does It Hurt, I think does it cause pain. So in the case of hurt and pain, they go hand in hand. If something hurts, it's by definition painful. Effectively they are one in the same, at least as it applies to rehab.
Soreness is the last word that came up. Soreness is interesting. Soreness is a response. We have muscle soreness which is a natural by product of early exercise and, is to be expected. Think of soreness as an after effect. If someone is doing an exercise and I ask Does It Hurt they sometimes say, “no it's just sore”. That might be semantical but I consider that pain and we stop or regress the exercise. 
For rehab purposes pain is immediate and soreness is residual. I have a standard soreness mini-speech I give to athletes and clients. It goes like this;
“ muscle soreness is good, normal and to be expected. Generally muscle soreness will be worse two days after exercise. The important thing to understand about muscle soreness is that it should be felt in the muscle. Pain in the tendon or the joint is not normal and should be considered a warning sign”
I usually go on to explain the areas where I don't want pain. I point out the anterior shoulder, the SI joint and patella tendons as important areas that we don't want to experience post workout soreness.
In any case, we should avoid pain, avoid exercises that hurt, probably tolerate a small amount of discomfort and, expect post exercise soreness. However, never try to make it a mental challenge. Never make it about how much you can tolerate. Painful exercise is a prescription for failure. I've rehabbed just about every injury known to man armed with some knowledge and the idea of Does It Hurt with great success and, I've seen the “ you need to expect a little pain “ philosophy fail over and over. Hope this helps.

The 9 Fun Practice Games Your Kids will Love!

Over the 38+ years of coaching baseball, all the baseball drills I used and all of the practice templates I have created, as well as camps and coaching different levels and ages of kids, I have learned some valuable things about how to run a baseball team.
I want my players to smile and have fun!
Youth Baseball Practice has to engage kids while helping them learn the game step by step. We encourage all youth baseball coaches to implement fun game in their practices that teach the kids how to compete, learn the parts of the game, and get them wanting to come back to practice! 
So below are some “MUST DOs” and some fun games for baseball practice. 
--Fun games MUST always incorporate fundamental drill techniques. Otherwise it is a waste of time for players and the coaches.
--Coaches should change their expectations slightly on a player by player basis depending on skill and ability.
--The younger the team, the more games there MUST be at practice (but make sure #1 is met).
--For older kids (13 years old +), incorporate games before and/or after the scheduled baseball drills – Kids are never too old for games! Just modify the priority of it in practice.
--Coaches should change out fun games each practice to keep kids engaged and practice different.

Youth Baseball Parent-Player Meeting

Congratulations, you have decided to coach a youth baseball team. Now it's time to meet with the parents and the players to set the tone for next season. Coaching youth baseball is a big responsibility and requires organization, planning and defined objectives. Requesting help from parents, and outlining communication. Player expectations need to be addressed as this is really the reason you’re coaching, for the kids.

This meeting may be the first opportunity for you to meet the parents and players who are anxious to learn what your knowledge base is, your coaching experience, and what style you use. Stating your objectives for the team and what it is you want to accomplish will put the parents and players at ease.

From my experience based upon youth feedback, youth baseball players want to experience success and have fun. As the coach you need to keep in that in mind, and so do the parents. But keep in mind that the reality is some parents view winning at all costs is the only thing that matters is their little superstar.

Explaining your coaching philosophy, rules for playing time, will it be equal playing time or based on performance, this will most likely depend on the age level you’re coaching, and player positions, how will you determine who plays where.

Parental involvement is sometimes glossed over. Since most kids state fun as their main priority, a close second is to be engaged with their parents. I stress the need with running practices, pre-game warm ups, and base coaches. Kids see this as an opportunity spend quality time with their parent. It is important that parents view it the same way.

You may want to comment for the benefit of the parents unwilling to participate on the field their assistance will be needed in other areas. In youth athletics there are numerous opportunities for parents to get involved by volunteering to coordinate any fund raising events, securing hotel rooms if your team is traveling out of town, will there be money that needs to be collected, what about the concession stand, so helping the team run smoothly is not limited to just the field.

Don’t over look the communication. Email is a great way for the Coach to disseminate information quickly, and for parents and players to keep the coach informed. As the coach you should informed the parents they can discuss anything with you, except playing time or positions. I have found this will eliminate the majority of your headaches if you state it like that.

Player expectation’s also needs to be expressed. Part of the learning process is how to take responsibility for themselves. Each player needs to be accountable for himself.
Baseball players need to think on their feet, react, adapt, overcome, by letting them know they, not their mother are responsible for insuring all their equipment is ready to go, is a great way to start teaching accountability. They further need to know there are consequences to not being accountable or improper behavior

Remember your job is to be the teacher, and at times a mentor. I would suggest that you start a list of items that need to be addressed at your parent/player meeting and add to it throughout the year as things come up. This will help you in future years to avoid any pit falls along the way.

Jerry Weinstein is an American professional baseball coach. He works for the Colorado Rockies of Major League Baseball.

Weinstein began his coaching career at Sacramento City College. He led the school to 831 wins across 23 seasons, and led the team to the National Junior College Athletic Association championship in 1998.

Weinstein managed the Gulf Coast Expos in 1993. He managed the Geneva Cubs in 1993 and Williamsport Cubs in 1994. After returning to Sacramento City College, he served as director of player development for the Los Angeles Dodgers in 2000 and 2001 and then coached for California State Polytechnic University, San Luis Obispo (Cal Poly) from 2001 through 2005. He left Cal Poly to become the manager of the Modesto Nuts in 2007 and managed them through 2011. The Rockies added Weinstein to their major league staff before the 2012 season.

After the 2013 season, the Rockies reassigned Weinstein, putting him in charge of player development in the minor leagues.

Pro athletes benefit from chiropractic care

Many American Pro athletes look at getting a Chiropractor's help in coping with the rigorous physical demands of their various sports. Professional golfers have access to Chiropractors during their tournaments, and many Pro football players are known to take Chiropractic assistance after strenuous games. Baseball teams have also started giving their players the much needed Chiropractic help.

In the previous year, baseball teams like the Boston Red Sox (World champions) and the Philadelphia Phillies have employed the services of Chiropractors to help their players perform well and remain in top notch condition. Clubs in the major league have to play 162 games as part of the schedule, and this includes a fair amount of travelling as well. Typical baseball seasons begin in late Feb and go on till early Oct. Teams looking at Playoffs or the World series have to push till late Oct.

The players of the Boston Red Sox can be seen with their Chiropractor, Michael Weinman, D.C, for 3 to 4 hours before they start their home games. He says that players can stay away from bothersome injuries and also perform at optimum levels when their muscles, joints, as well as nerves, are all in top condition. He also says that adjustments made by a Chiropractor can also better one's overall physical health and thereby result in excellent performances.

Injuries that are common in baseball players include mid & low back injury, neck injury, rotator-cuff injury, and also hamstring injury. Hamstring related problems normally start because of players having to suddenly leap into action fast from a standing position while making their plays.

Most of Dr. Weinman's work involves helping thwart inflammations, breaking up adhesions, and adjusting various joints to enhance mobility. He, along with his trainers, physical therapists, and massage therapists look to have had a good effect on the team, as they won against the Colorado Rookies in four straight games, to win the World Series.

The Phillies are now to have a Chiropractors network in all the cities where they are scheduled to play their games (home and away). The first player in the team who requested the presence of Chiropractors was the pitcher, Cole Hammels. He is of the opinion that he missed out on games in 2007 (being on the disabled list) because he did not get Chiropractic assistance for his elbow strain.

Hamels also said that he was very happy with the knowledge that the team was making efforts to ensure that they get Chiropractic assistance while they are paying in different cities. Other players in the team are also likely to take the assistance of the Chiropractors in coping with the adjusting extremities. He added that owing to consulting a Chiropractor he has surely benefited, and that it was great that the team was going out of their way to provide the players with this assistance. According to Hamels, the team could very well get to be a 20 game winner in this season, thanks to the Chiropractic services that are now available to all the players of the team.
